CONCLUSION
In this text I have attempted to present a management and treatment philosophy. Since there are entire textbooks on the subject matter of many of the chapters of this text, my objective has not been to write an exhaustive text, but rather to present what has been for me a successful approach to the management of difficult cases. The approach and techniques described are entirely applicable to routine use in a practice environment. All of the cases presented in this text have been photographed in such an environment and none have been especially prepared for this publication.
