Post-traumatic zygoma and orbital walls reconstruction: integration between surgical navigation and stereolitographic models

Background and objectives: The correction of post traumatic orbital and zygoma deformity is an hard challenge for maxillo-facial surgeons. The integration of different technologies such as software, surgical navigation and stereolithography, opens new developments in terms of choice of the reconstruction for each individual case. The purpose of this study is to analyze the obtained results, in order to verify suitability, effectiveness and reproducibility of the used protocol.

Materials and methods: Ten patients were included into the study. Inclusion criteria: (a) unilateral pathology, (b) no syndromic patients, (c) association with diplopia and enophthalmos or esophthalmos, zigomatic deformities. Pre-surgical planning was performed with iPlan 3.0 CMF ® software and surgical navigation performed by Vector Vision II (BrainLab ® ). We used 1:1 skull stereolithographic models for all the patients. Orbital reconstructions were performed by preformed orbital mesh.

Results: The results are referred to: (a) correction of deformities, enophtalmos and diplopia, (b) correspondence between reconstruction mesh position and mirroring of preoperative planning, (c) difference between the reconstructed orbital volume and the healthy orbital volume.

Conclusion: The proposed protocol incorporates all the latest technologies to plan in every detail the virtual reconstruction surgery. The results obtained from our experience are very encouraging and lead us to pursue this path.
