Minimally invasive implant insertion based on computer supported implant planning

Purpose: Taking into account the increased expectations of our patients regarding immediate postoperative life quality minimally invasive implant insertion is discussed frequently among experts. The major challenge of this technique, however, is the reduced view of the bony implant site. Taking this into account it was the aim of the present case series to investigate the efficiency of an indirect DVT based implant planning system.

Methods: Patients of our department underwent preoperative digital volume tomography (DVT) scanning with a splint containing the missing teeth as a bariumsulfat model. X-ray splint and DVT data were submitted to an implant planning center. Here the ideal implant position was determined and transferred to the X-ray splint using the CAD/CAM technology. Implant insertion was performed using the minimally invasive punch for uncovering of the alveolar crest. Postoperatively the implant position was controlled using the orthopantomogram (OPT).

Results: In all patients implants were inserted in the prosthodontically ideal position using the minimally invasive approach. Intraoperative complications, like vestibular perforation, as well as postoperative complications, like infection due to soft tissue contamination of the implant during insertion, were not detected.

Conclusion: Computer-based planning seems to reduce the complications of minimally invasive insertion und might have the potential to contribute to an expansion of the indications of the minimally invasive technique.
