The techniques used can be divided as follows:

1 Surgery to eliminate disease and produce conditions to minimise its recurrence (outlined in this chapter):

• Pocket elimination or reduction:
• gingivectomy (Fig. 25.1);
• apically repositioned flap (Fig. 25.2).
• Accessing root for cleaning:
• replaced flap (modified Widman flap) (Fig. 25.3).
• Distal wedge excision and tuberosity reduction (Fig. 25.4).

2 Surgery to eliminate disease and regenerate lost periodontal structures (Chapter 26):

Guided tissue regeneration.

• Enamel matrix derivative (Emdogain®, Biora).
• Bone grafting.

3 Surgery for root coverage (mucogingival surgery) (Chapter 32):

• Pedicle grafts:
• rotational flaps: laterally repositioned flap or double papilla flap;
• advanced flaps: coronally advanced flap or coronally advanced flap plus enamel matrix derivative.
• Free epithelialised gingival grafts.
• Guided tissue regeneration.
• Subepithelial connective tissue graft.
