Management of sinus membrane perforation in maxillary sinus graft procedures

The correct and precise understandings of the applied anatomy, physiology, and pathology about the maxillary sinus cavity can provide the successful procedures for the maxillary sinus surgery. Above all, it is important that we have to understand the osteomeatal complex and the patency of maxillary ostium is pivotal in maintaining the physiology of maxillary sinus cavity. I want to review some complication cases which were referred from local clinics after sinus graft procedures, and to present the clinical importance of the patency of maxillary ostium for the successful maxillary sinus surgery procedures.

During the maxillary sinus surgery, there might be some complications as follows: improper window design & formation, sinus membrane perforation or tearing, bleeding and so on. Among them, the sinus membrane perforation or tearing might be most common. I usually repaired such perforation or tearing with tissue adhesive (fibrin glue, Beriplast ® ). Because the tissue adhesive is very convenient & successful for the repair of sinus membrane perforation or tearing and for the fixation of bone graft materials, I will present & recommend the use of tissue adhesive with some clinical cases reviews. Also, I want to review the complications during maxillar sinus surgery, and present some know-hows for the treatment and prevention of the complications in maxillary sinus surgery procedures.
