Background and objectives: Rehabilitation of atrophic ridge for both surgeon and prosthodontist always being a matter of concern. The aim of this article is to present a new methodology based on a forgotten approach to rehabilitation of a severely absorbed mandible.

Methods: A 55-year-old women with severe mandibular atrophic ridge was selected. A CT scan which was met the standard of computer based navigation system was taken under leading a specially designed scannographic guide. The Raw data of CT, were transferred in an specially dedicated software program (SIMPLANT™, Materialise Dental, Belgium) and the design of implants were derived in accordance with the implant placement technique initially introduced by tatum (named Ramus Frame). The designing were verified by surgeon and prosthodontist in a same session.After designing the site of the implants a surgical guide was made according to manufacturer ordering procedure. A total of four dental implants were installed via flapless technique. After 3 months the impressions has been made and an specially designed prosthesis has been fabricated.

Results: Post operative showed favourable design after 6 months follow up. No sign of Pain or discomfort has been reported by patient and scheduled radiographic examination showed implant survival. No negative bone index has been shown.

Conclusion: This firstly reported clinical case showed a ramus area could be a satisfactory donor site for dental implant placement in selected atrophic ridge.
