Procedure 20-1 DENTAL HYGIENE CARE PLANNING

STEPS RATIONALES
1. Link care plan to dental hygiene diagnoses. The dental hygiene diagnosis is the foundation of the dental hygiene care plan.
2. Establish priorities of need. Client’s unmet human needs are ranked.
3. Set client-centered goals. Addresses signs and symptoms supporting the dental hygiene diagnosis and client’s desired outcome of care; also specifies the time and method of evaluation.
4. Select dental hygiene interventions. Selected to address the contributing factors of the dental hygiene diagnosis; means to achieving the desired outcome of care.
5. Establish an appointment schedule. Schedules the interventions that will most likely achieve the desired outcome of care.
