Local Anesthesia for the Trigeminal Nerve (CN V)

  • Complete list of cranial nerve innervations on pages 31–34
  • Trigeminal nerve sensory distribution (Figure 9.6)
A schematic diagram of a trigeminal nerve sensory distribution, reads pink colour region for V subscript 3, red colour for C subscript 2 per C subscript 3, blue for V subscript 1 and V subscript 2.

Ophthalmic Nerve (V1)

  • Smallest of the branches
  • Innervation
    • Sensory only:
      • Scalp
      • Eye
      • Nose
      • Forehead
  • Supraorbital Nerve Block
    • Palpate the foramen
      • Intersection of the medial one‐third and the lateral two‐thirds of the orbit, usually in the sagittal plane with the pupil
    • Create a wheal with 0.5–1 ml of anesthetic solution
    • Apply firm pressure
  • Supratrochlear Nerve Block
    • Can be blocked immediately after the supraorbital by advancing the needle 1 cm toward the midline and injecting additional 0.5–1 ml of anesthetic solution
    • Apply firm pressure after each block

Maxillary Nerve (V2)

  • Innervation
    • Sensory only (Figure 9.7)
      • Inferior orbit
      • Nasal cavity
      • Maxillary teeth and soft tissues
      • Maxillary sinus
