ASA Physical Status Classification

  • The American Society of Anesthesiologists (ASA) assigns a numerical score to help assess and communicate the overall health status and perioperative risk
  • Examples from the ASA are listed below

ASA I

  • Healthy individual
  • No medical problems
  • Normal BMI

ASA III

  • Moderate‐to‐severe systemic disease
  • Autism with severe limitations
  • Substantive functional limitations
  • Poorly controlled asthma
  • COPD
  • Poorly controlled hypertension
  • Poorly controlled diabetes mellitus
  • Insulin dependent diabetes mellitus (IDDM)
  • Active hepatitis/alcohol dependence
  • Morbid obesity (BMI ≥ 40)
  • Previous myocardial infarction with or without CAD/stents (>3 months)
  • Previous stroke/TIA (>3 months)
  • Controlled CHF
  • Implanted pacemaker
  • ESRD undergoing regular dialysis
  • Difficult airway
  • Severe OSA

ASA V

  • A moribund patient who is not expected to survive without the operation
  • Trauma
  • Ruptured TAAA
  • Multiple organ system dysfunction
