Reduction and internal fixation of zygomatic arch and malar bone fractures endoscopically supported: a protocol proposal for treatment

The reduction and internal fixation of unstable fractures which affect the region of the zygomatic arch and malar bone is no a small challenge to the maxillofacial surgeon who works in trauma team.

Often, in addition to the biomechanical instability these fractures incur, the time between the traumatic event and the timing for treatment must be considered. This delay is often associated with severe neurological or acute illness, preventing an immediate resolution. Consequently, the approaches recommended for making the open reductions are extensive and may be associated with significant cosmetic sequelae.

This work presents a group of patients treated with a protocol treatment developed by the Carlos Van Buren Hospital’s Unit of Maxillofacial Surgery, for unstable fractures that were necessarily delayed (over 15 days). The reduction and osteosynthesis proposed protocol of the zygomatic arch and malar bone it was through anterior and preauricular acces, endoscopically supported.
