Endoscopic temporalis muscle flap in maxillo-facial surgery

In recent years we have developed increasing interest in endoscopy and minimally invasive surgery. Thus we decided to transpose our experience in aesthetic surgery in daily clinical practice and oncology using an endoscope to dissect the temporalis muscle flap for both orbital or maxillary reconstruction. We believed that minimal dissection could avoid or decrease the post-op retraction in the temporal region since we mantain the attachment of the soft tissues above the muscle to the parietal bone and to the zygoma. Initially dissection took about 1 h and an half but proceeding with time the dissection become faster decreasing to 45 min. The retraction of the skin in the temporal region seems to be lower than the conventional surgery even if only 1 year has passed from the fist surgery. We believe that a kind of prosthesis must be developed in order to cancel any kind of asimmetry. We confirm that this endoscopic procedure is a minimal invasive surgical prcedure and a good option to dissect a viable muscular flap for maxillo-facial reconstruction. No facial nerve impairment was ever seen in the cases treated in our Department.
