Concomitant treatment of mandibular ameloblastoma and temporomandibular joint (TMJ) severe osteoarthritis with bone graft and total joint protheses (TJP)

Background and objectives: Solid amelobastoma affects many patients and studies emphasize that inadequate treatment result in recurrence. Patients with TMJ pathologies may benefit from TMJ reconstruction with TJP to achieve the best outcome. This paper aims to present the treatment planning and outcomes of a patient affected by recurrent ameloblastoma associated with end-stage TMJ dysfunction caused by osteoarthritis.

Methods: The treatment of the ameloblastoma on the body of the mandible consisted of mandibular resection and immediate reconstruction with bone graft from the iliac crest. The bilateral TMJ pathology was treated using TMJ concepts custom made TJP. The prosthesis in one side had an extension from the TMJ area to the pre molar on the opposite side. The bone graft at the ressected area was secured with screws on the prosthesis.

Results: The results are favourable after 2 years of follow up. The patient is undergoing oral rehabilitation for the installation of dental implants. Shows good functional and esthetic results.

Conclusions: We conclude that the concomitant treatment with resection of an ameloblastoma followed by bone graft reconstruction associated with TJP could be a predictable treatment.
